Please bring your insurance cards with you to your visit.
Our staff is experienced in working with vision insurance providers. We will submit claims directly to your vision insurer. Most plans often require a co-payment for an office appointment. Co-payments are the responsibility of the patient and are due at the time of the exam.

What if I my insurance company didn’t provide a card?
In those instances, you will need to know the name of the vision insurance company, the subscribers or plan holder’s social security number and date of birth.

Flexible Spending Accounts and Health Savings Accounts
Many employers offer a Flexible Spending Account (FSA) or Health Savings Account (HSA). Any out-of-pocket expenses incurred at our office are eligible to be paid for with a FSA or HSA.
